New patient appointments will be made after the completion of returned forms.
Please select the physician with whom you would like to schedule, and complete the appropriate forms.

Please send a copy of the front and back of your insurance card to
atcforms@southbayarthritis.com

Because we do not accept all insurances please confirm with your insurance that we accept it. We do accept HMO insurance with Torrance IPA and Axminister only. With prior authorization to schedule an appointment. We do not accept any workers’ compensation claims or auto accident insurance claims.

Once forms are submitted a member of our staff will contact you to schedule the next available appointment.
